How does an operator know that the barriers against a major accident work today, and not just on paper? In this advanced course on process safety management, you follow the team of the FPSO Horizonte Azul, a fictitious deepwater facility, through the whole system: the elements and pillars of a management system, leadership, culture and ILO Convention No. 174; hazard identification with HAZID and a simulated HAZOP session; major accident risk assessment, layer of protection analysis (LOPA) and ALARP demonstration; building bow-ties, types of barrier, safety-critical equipment, performance standards and verification; management of change, pre-startup safety review and the control of bypasses and inhibited alarms; Tier 1 to 4 indicators and learning from accidents. It ends with an integrated assessment of an FPSO's system, with priorities and an improvement plan. Includes downloadable spreadsheets and templates. Aligned with IOGP Report 456, ISO 17776, IEC 61882 and ILO Convention C174; it does not, on its own, qualify you to facilitate HAZOP studies or to approve risk studies. The course is delivered in Portuguese.

What you will learn

• Describe the elements of a process safety management system, the role of leadership and culture and the requirements of ILO Convention C174.
• Prepare and run a simulated HAZOP session and distinguish when to use HAZID and other techniques.
• Assess major accident risks with the hierarchy of controls, consequence analysis and LOPA, and demonstrate ALARP.
• Build a bow-tie, classify barriers according to IOGP Report 544 and define performance standards and barrier verification.
• Apply management of change, pre-startup safety review and the control of bypasses and inhibited alarms.
• Build a Tier 1 to 4 process safety indicator dashboard and link it to learning.
• Assess a facility's process safety system and propose priorities and an improvement plan.
