After an accident, finding someone to blame is easy; preventing it from happening again takes method. In this course for supervisors, HSE technicians and investigation team members, you follow the investigation of an accident on a conveyor belt at a fictitious mining company, from the first night to the lessons learned. Learn to distinguish between an accident, a near miss, an undesired circumstance and a dangerous occurrence, to comply with the notification required by the applicable law and to decide the level of investigation by potential, in a just culture. Organise the immediate response and gather reliable evidence (people, positions, parts and paper, photographs, sketches and electronic data), with a chain of custody. Conduct interviews with open questions and without blame, and build the incident timeline. Identify immediate, underlying and root causes with the 5 Whys, the cause tree and the Ishikawa diagram, without stopping at "human error". Finally, set SMART corrective actions using the hierarchy of controls, write the report, check effectiveness and share safety alerts. Aligned with UK HSE guide HSG245, ILO guidance and ISO 45001. Includes downloadable templates: investigation level matrix, evidence gathering checklist, interview guide, timeline template, cause analysis templates, report, action plan and safety alert. The course is delivered in Portuguese.
