Many of the most serious industrial accidents happen during maintenance, when someone opens a line they thought was empty or works on a machine that suddenly starts up. In this course for permit issuers and receivers, supervisors and maintenance technicians, you follow the team at a fictitious gas processing plant. Understand the purpose and components of a permit to work system and the lesson of Piper Alpha on permits and shift handover. Follow the life cycle of a permit, from request to close-out, with the roles of the requester, the issuer, the area authority, the performing authority and the isolation authority, the JSA, supplementary certificates, the site visit, suspension, shift handover and simultaneous operations. Apply the steps of isolation and lockout of hazardous energy (LOTO): prepare, shut down, isolate, lock and tag, dissipate and verify, with group lockout and a lock box. Finally, recognise typical failures, stop and reassess when conditions change, and learn about field checks and system audits. Aligned with the British HSE guides HSG250 and HSG253 and the IOGP Life-Saving Rules. This course does not authorise anyone to issue or accept permits or to carry out isolations: that requires formal authorisation by the site and a competence assessment. The course is delivered in Portuguese.
